Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of crushing, struck by and amputation: a) On or about 7/2/07, in the Bottle Line area, employees operate the EPOC Wastebaler. The employer did not assure that employees were adequately trained in, understood, and followed safe operating procedures for the baler. b) On or about 7/2/07, in the Bottle Line area, there was no inspection and maintenance program in place for the baler. c) On or about 7/2/07, in the Bottle Line area, records of reports of malfunctions were not maintained. Acceptable abatement methods to correct the hazards are: a) Train and instruct employees to not put hands and arms in baler for any reason. Train employees to properly lockout/tagout and properly block the ram while clearing and unjamming, in accordance with CFR Part 1910.147 and the manufacturer's instructions. b) Training employees on operation of baler in accordance ANSI Z245.2-1992, Section 6.1(b). c) Implement an inspection and maintenance program for the baler in accordance with ANSI Z245.2-1992 Section 6.1(e). d) Maintain records and reports of malfunctions in accordance with Section 6.1(m); and e) Keep all surrounding walking areas and floors free from obstructions, accumulations of waste matter, grease, oil and water in accordance with Section 6.1(l) of ANSI Z245.2-1992.
