Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: On or about 12/28/07, employees erecting tie-beam formwork from a job-made work platform that was elevated on the forks of a Traverse Lift Lull were exposed to an approximate 15 foot fall hazard. The work platform did not comply with The American Society of Mechanical Engineers, ASME B56.2-2002, "Safety Standard For Rough Terrain Forklift Trucks" as it was not ASME/manufacturer approved, was not secured to the lull forks, and did not have an attachment point for a full body harness. Among other methods, one feasible and acceptable method to abate this hazard is to use an ASME/manufacturer approved work platform attachment for the lull.hment
