Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to struck- by and crushing hazard: a) Loading Dock, Publix Super Markets, Inc. Deerfield Beach, Florida. On or about 04/09/2007, at Bay door #218, the back up alarm on the OTTAWA # 934 Tractor was not functional, exposing employees to the hazards of being run over, struck by, and or crushed by the equipment. Among other methods, one feasible and acceptable method of abatement to correct the hazard is to implement a policy that all the back up alarm on the OTTAWA Tractors are functional. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
