Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Nevada Revised Statute 618.375(1): The employer failed to furnish employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees: The employer failed to furnish employment free from recognized hazards as follows: (a)At the CityCenter project, Pelli Tower, employees operated and performed lubrication maintenance on the Hercules manlifts in use. The employees performed the lubrication work without shutting down and locking out both cars on the manlifts, and operated the cars to transport workers prior to performing the daily inspections, against manufacturer's requirements. One employee lubricated parts on his car daily. The employees were exposed to possible serious injuries or death while performing the lubrication maintenance and operating the lifts. Reference: Hercules Elevators Safety Rules: Item 2: DO NOT CARRY PASSENGERS until the daily inspection is complete. Inspect the hoist daily in accordance with the inspection schedule. Item 9: DISCONNECT POWER AND LOCK OUT DISCONNECT SWITCH(ES) (with padlocks), BEFORE LUBRICATING THE HOIST CAR(S). (On twin hoists both disconnects must be locked out before lubricating one or both cars). Under no circumstances should a car be electrically energized while greasing car(s). Reference: Hercules F Series Manual Chapter 9 - Lubrication, p. 9-1 and 9-2. Lubrication shall be carried out according to the period listed in the table below. (Table lists Weekly, Monthly, Quarterly, and Biannually lubricating requirements - none daily). One feasible means of abatement would be to ensure employees follow the manufacturer's Safety Rules and lubrication schedule.
