Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: Drilling floor; on or about March 27, 2008, employees engaged in the drilling of an oil/gas well with a drilling rig used the break out tong during pipe connections and tripping out of the well (removal of the drill pipe string); the wire rope connecting the tong to draw works was severely damaged and had an knot for attachment. This exposed the employees to the hazard of being struck by an uncontrolled tong assembly upon failure of the wire rope. Among other methods, feasible and acceptable abatement methods to correct this hazard would be to conduct wire rope inspections and removal criteria as recommended by American Petroleum Institute (API) Recommended Practice (RP) 54, Section 9.6; and remove damaged wire ropes from service as mandated by RP 54 Section 9.6.6. Additionally, RP 54, Section 9.9.5, assure that fittings to attach the back up lines have a minimum breaking strength at least equal to that of the wire rope.
