Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5 (a) (1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazards of lacerations, impalement, and crushed- by injuries associated with heavy equipment: In the equipment yard: On or about March 28, 2008, employees were not provided the proper instructions to safely connect dollies to trucks in preparation for over-the-road travel. This condition exposed employees to the hazard of being struck-by moving equipment. A feasible method to eliminate or reduce the hazard: A feasible method to eliminate the hazard would be to adopt the manufactures recommendation on boom connecting procedures. The boom connecting procedure requires the boom to be secured to the dolly before the air lines are connected. By connecting the boom to the dolly first, there is no air flowing to the dolly braking system, "which disengages the brakes". When the boom is connected to the dolly, this prevents movement of the dolly when the air lines are connected.
