P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of a flash fire: a) Building "F" - The employer failed to ensure that employees installed components including fittings and hoses on a four bottle oxygen cart that were cleaned to a degree required for the safe operation of oxygen service equipment. Combustible material inside the manifold ignited in the presence of pure oxygen at 2000 p.s.i., resulting in the failure of the manifold and a flash fire. Among other methods, feasible and acceptable means of abatement is to use components cleaned for oxygen service in accordance with the Society of Automotive Engineers, Aerospace Recommended Practice 1176 Oxygen System and Component Cleaning and Packaging.
