Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries in the event of a forklift tip-over or other accident. a) On or about May 16, 2007 in the lumber receiving and storage areas - The operator of a Taylor TYB 180 M forklift was not wearing a seat belt while unloading and stacking rough cut lumber. Feasible means of abatement include; observe powered industrial truck operations to determine if trucks are being operated safely, and conduct employer/employee interviews to verify training program implementation. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
