Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall or burn hazard: On or about March 29, 2007, and at other times prior thereto, between the rig monkeyboard to ground level, the auxiliary emergency escape system was installed in an unsafe manner and included the following visible deficiencies: a.The auxiliary escape line was hooked to the anchor point with a chain connected by a hook and not clamped per the operator manual; b.The ground anchor for the auxiliary escape line was not installed per the operator manual. The employer permitted the use of a free-standing barrel of concrete as the termination ground anchor; c.Crosby fist grip clamps were installed not evenly spaced as per the operator manual. Among other methods, one feasible and acceptable abatement method involves the employer ensuring the emergency escape line system is installed in accordance with Section 6.10 of the American Petroleum Institute's Recommended Practice 54 (3rd Edition, 1999) as well as Crosby clip and manufacturer's instructions.
