Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to failure of the emergency escape line and falling to ground level: On 21 February 2007, and at other times prior thereto, employees working on the derrick platform were exposed to hazards associated with failure of the emergency escape line, such as, but not limited to, falls in the event of emergency escape. b.The wire rope used for the emergency escape device was not continuous. The wire rope was connected to a chain which in turn was wrapped around a stake driven into the ground. c.The anchorage for the emergency escape device was a metal stake driven into the ground. Among other methods of abatement, one feasible and acceptable abatement method to correct this hazard would be installation of a properly rigged and secured emergency escape line in accordance with the manufacturer's recommendations and API Recommended Practice 54, Third Edition, August 1999, Reaffirmed March 2007, Section 6-10.1.
