Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Receiving area, on or about 01/17/2002: Two Jet, 1 ton, overhead electric hoists, Serial Numbers unknown, had the safety latches removed and had no maintenance records to indicate annual or more frequent inspections. Among other methods, feasible and acceptable abatement methods to correct this hazard are to: 1.Conduct frequent and periodic inspections of all hoisting equipment and repair or replace broken, defective or missing components. 2.Maintain dated inspection reports or comparable records on critical items such as hoisting machinery and other lifting devices in accordance with ANSI B30.16-1993, Overhead Hoists, Chapter 16-1.2.9 Hooks and Chapter 16-2 Inspections and Maintenance. b)Receiving area, on or about 01/17/2002: Two overhead monorail beams holding 2-Jet, 1 ton, overhead electric hoists were never initially load tested, rated and marked and no frequent or periodic inspections have been conducted or maintained. Among other methods, feasible and acceptable abatement methods to correct this hazard are to: 1.Prior to initial use, all new, reinstalled, or modified equipment shall be tested to assure compliance with ANSI B30.11-1980, Monorail Systems. 2.Conduct frequent and periodic inspections of all hoisting equipment and repair or replace broken, defective or missing components. 2.Maintain dated inspection reports or comparable records on critical items such as hoisting machinery and other lifting devices in accordance with ANSI B30.11-1980, Monorail Systems, Chapter 11-2 through 11-2.1.5 and 11-2.3.3. Abatement certification must be submitted for these items.
