Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to crushing injury from collapse of the crane: a) On or about June 8, 2007 and times prior, employees were exposed to crushing injury while using a truck mounted telescoping crane in which the outriggers were set upon 2x6s (wood boards 2-3 feet long) that were not uniform or adequate in size to prevent unintended tipping or collapse of the crane. as referenced in ASME B30.5- 2000. Among other methods, one feasible and acceptable abatement method to correct this hazard is: To follow mehod as referenced in ASME B30.5-2000 Section 5-3.2.1.4 Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
