Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a)Jobsite - on or about August 15, 2007 - an employee sustained fractures after a fall of approximately 30 feet while engaged in the placement of concrete, guiding the delivery of the concrete with the end hose of the concrete pump truck. The employee was working adjacent to the end of the concrete pump hose while the pump was started and restarted. This is a violation of The Concrete Pump Manufacturers Association procedures for safe operation, Safety Standard for Concrete Pumps, Placing Booms and Delivery Systems, Section 27-1.18.2.2 Methods to correct this hazard include, but are not limited to: 1) Develop and implement a comprehensive training program for employees working in the vicinity of this equipment. 2) Follow the procedures outlined in the Concrete Pump Manufacturers Association's Safety Standard, under the heading of Operator Information, 27-1.18.2.2 paragraph# 10, which states; "Personnel should be cleared from the area of the end hose when starting or restarting the pump". ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM.
