Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck by steel flywheel while removing the pitman of a moveable jaw rock crusher: a) Worksite located at 625 Mount Hope Road, Rockaway, NJ An employee was fatally injured when struck by a 57 inch steel flywheel, of a movable jaw rock crusher, that was attached to the pitman during removal. Violation disclosed on or about 10/25/06 AMONG OTHERS, FEASIBLE AND REASONABLE METHODS OF ABATEMENT ARE TO: 1) Follow the removal procedures per the Parts, Operation and Maintenance Manual for the Cedarapids Jaw Crusher Model #2248 and 2) Provide employees with written instructions outlining the tasks to be performed and hazards to be avoided (i.e. per a Job Hazard Analysis, Standard Operating Procedures).
