Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which are free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: An employee was exposed to a struck by hazard from a threaded pipe under pressure. On or about 5/1/2007,an employee was bleeding pressure off the discharge line of a compressor when the pipe sheared off at the threads and struck the employee. The approximate pressure was about 900 psig. The Hadley 1H wellhead is near the intersection of CR 1236 and FM 200, approximately 100 yards off FM 200 in Johnson County, Texas. Among other methods, one feasible and acceptable abatement method to correct this hazard is to substantially brace or support the pipe to protect against excessive stresses arising from pressure, settlement, vibration, expanison, or contraction.
