Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling and being crushed by a forklift: On or about 09/27/06, at berth 31, an employee who was operating a powered industrial truck in reverse direction with the view of the operator obstructed and without wearing a seatbelt, was struck-by a semi-truck that was traveling in the forward direction, exposing the employee to the hazards of falling from the forklift and being crushed by the trucks. Among other methods of abatement, one feasible and acceptable method to correct these hazards is to prohibit employees from operating this type of equipment with items that could obstruct the view of the operator to the rear and prohibiting employees from using this type of equipment without wearing seatbelts.
