Section 5(a)(1) of the Occupational Safety and Health act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious harm to employee in that employees were exposed to crushing hazards: Pallet Yard: An employee operating the Hyster forklift serial number A187V11487J was not wearing the provided seatbelt. Among other methods, one feasible and useful method to correct this hazard is to require and enforce the use of the seatbelt when the forklift is operated.
