Text For Citation: 01 Item/Group: 001 Hazard: LACKTRAIN
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a/ Evacuation of employees from an elevator car was not performed under the direct supervision of trained elevator personnel, and a trained rescue team had not been formed prior to conducting a rescue of employees from a stalled elevator. While ultimate responsibility for correcting the hazards rests with the employer, given his/her superior knowledge of the work place, feasible and acceptable methods of abatement to correct this hazard, among others are: 1. Use trained Elevator Personnel and follow the guidelines of Part I, Section 1.1 of the American Society of Mechanical Engineers Guideline for Emergency Personnel, ASME A17.4-1999. Use trained elevator personnel who have been trained on proper evacuation procedures for removing people from an elevator as outlined in Part I, Evacuation Procedures. 2. Have building management select and train their on employees in the proper evacuation procedures per Part I, Section 1.2.1 of the American Society of Mechanical Engineers Guideline for Emergency Personnel, ASME A17.4-1999 as outlined in Part I, Evacuation Procedures. 3. Contact emergency services of the local fire department and follow the guidelines of Part II of the American Society of Mechanical Engineers Guideline for Emergency Personnel, ASME A17.4-1999. Use trained emergency response personnel who have been trained on proper evacuation procedures for removing people from an elevator as outlined in Part II, Evacuation Procedures. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS VIOLATION. The documentation should include written verification of abatement, applicable measurements of monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).n
