Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: TETRA Production Testing Services, L.P., Section 4 Blk 20, University No 20-4 No 1, Mentone, TX. For the period of time up to and including 06/24/06, the employer failed to ensure that pressure relief devices were used to protect employees working around/on high pressure vessels such as, but not limited to, line heaters. Feasible and acceptable abatement methods include: Install pressure relief valves where required by the hazard to protect against overpressurization of the line heaters and three-phase separators.
