North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)Facility - - Assembly Area: The pedestal-mounted Contrx jib crane with the Jet 1-Ton manual chain hoist had not received a rated load test. b)Facility - - Machine Shop: The pedestal-mounted Gorbel jib crane with the Coffing 1-Ton electric chain hoist had not received a rated load test. REFERENCE:ANSI/ASME B.30.11-1998: Monorails and Underhung Cranes: Chapter 11-2: Inspection, Testing, and Maintenance: Section 11-2.2: Testing: 11-2.2.2 Load Test: a)New, re-installed, altered, repaired, and modified equipment should be load tested prior to initial use as determined by a qualified person. c)If a load test is conducted, the person conducting the load test shall prepare a written report of the load sustained during the test and the operations performed during the test. Report shall be placed on file. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to have a rated load test of each jib crane/hoist system at the facility performed by a competent person and to maintain the documentation generated from the load tests in a permanent file at the facility for the operational life of the hoist systems.nal
