Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of explosions and/or steam eruptions in the slag pits. On or about April 9, 2006, at the Blast Furnace, precautions and training were inadequate to ensure that water did not accumulate in the East #1 slag pit and precautions and training were inadequate to prevent large amounts of molten iron from running into the East #1 slag pit. Among other methods, one feasible and acceptable abatement method to correct this hazard is to develop, train on and enforce a procedure to prepare for and anticipate the possible failure of the "Rose" dam and contingency procedures if failure of the "Rose" dam should occur, plus develop and enforce a procedure to ensure that water does not accumulate in the slag pits.
