Text For Citation: 01 Item/Group: 001 Hazard: LIFTING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to death and serious injuries from a substandard suspended unguarded platform. Elevated work platforms used with Rough Terrain Forklift as designated by the manufacturer must meet design, construction, and use requirements as outlined by American National Standard Institute ASME/ANSI B56.6 2002,Safety Standards for Rough Terrain Forklift Trucks section 8.24.1(b): Platform width was greater than 10 inches past the width of the load bearing tires on either side. Also ASME B56.6 2002 section 5.15.1 (b): be certain that the platform is securely attached to the lifting carriage and forks. a) The work platform extended more than 10 inches past the load bearing tires on each side. b) The platform was not securely attached to the lifting carriage and forks. Among other methods, feasible and acceptable means to correct these hazards are to use a safer alternate method such as the use of a platform approved by the manufacture meeting the requirement of ASME/ANSI B56.6-2002 for the attachments on rough terrain lifts and ensure the appropriate size for the working platform. Abatement Certification and Abatement Documentation required.
