Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: Rig #20 well site: On or about March 2, 2006, employees engaged in the drilling of a gas well were exposed to the hazard of being struck by head-block and drill string assembly when the brake systems on the main hoist line failed to hold the load and came crashing down on the drill floor. The current brake inspection procedures failed to detect that the electric brake was uncoupled from the draw works because the latch holding in the brake engagement handle fell off when its hinge nut became loose. Among feasible and useful methods to correct the hazard of having a brake system fail would include: a)The brake inspection procedures must comply, at a minimum, with the Manufacturer's requirements found in the Nation-Varco manual for this system. Brake inspections must be conducted by competent persons. b)In order to reduce stress on the brake system and eliminate overheating, a safe descent speed should be established at less than 300 feet per minute as recommended by the Nation-Varco manual for this system.
