Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling suspended loads: On or about March 14, 2006, at this facility employees were exposed to the hazard of being struck by falling suspended loads while using a Campbell SAC clamp to hoist a load. The employer did not ensure the following with respect to the use of the clamp: (a) Employees were not kept clear of the suspended load; (b) The Campbell SAC clamp was not used or maintained in accordance with manufacturer's written instructions; (c) Periodic inspections of the SAC clamp were not documented; and (d) The markings and serial number of the Campbell SAC clamp were defaced and unreadable.
