Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: At this inspection site -- Due to failure to inspect and maintain records on the derrick, mast, four-inch nipple, hammer connection unions at the gooseneck on the standpipe, gooseneck on the swivel and the rotary hose and their auxiliary parts in safe condition as specified by API RP 54-92, Section 8, paragraph 8.2.1, employees were exposed to a struck-by hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement a maintenance and inspection program as addressed in the International Association of Drilling Contractors (IADC) Drilling Manual and the American Petroleum Institute (API) RP 54-92.
