Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling loads and/or falling hoist parts: a)Preval Department - PSM Covered Process, Gassing House: A shop-made underhung hoist was being used to lift the metering cylinders out of the propellant filling head equipment and the following deficiencies were found to be present on or about 7/18/07: (1) Initial load test was not conducted with a written report to confirm the hoist's load rating; (2) Rated load capacity was not permanently marked on the hoist; (3) The hook did not have a safety latch; (4) The hoist components had not been subject to periodic inspections. One feasible and acceptable abatement method, among others, to correct this hazard is to follow the testing, marking and inspection requirements referenced in ANSI/ASME B30.16-2003. NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.ce
