Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught in and/or struck by hazards: (a)Cross Member Assembly Area, W-450 Welding Operation, Control Console: On March 7, 2006, an employee was stuck by and crushed by a robot, while performing welding operations. The employer failed to ensure that the emergency stop button was operational. The emergency stop button was missing the red mushroom cap and the yellow background. (b)Cross Member Assembly Area, W-450 Welding Operation, Control Console: Employees were exposed to struck by hazards, while performing welding operations. A metal cylindrical object (section of pipe) was positioned around the emergency stop button, which impeded the accessibility of the emergency stop function. Among other methods, one feasible method is to comply with ANSI/RIA R15.06-1999, Section 4, Paragraph 4.6.2 Emergency stop devices and 4.6.3 Emergency stop device design, which states that "each operator control station, including pendants, capable of initiating robot motion, shall have a manually initiated emergency stop device". The push button that activates an emergency stop circuit shall be "red in color with a yellow background, unguarded, and have palm or mushroom head type".
