Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed in the event of an upset or overturn of a forklift: The employer does not ensure that each powered industrial truck operator is protected from the hazard of being crushed in the event of an upset or overturn of a forklift. This violation occurred observed on October 31, 2007 at a worksite located at 56 Pine Street, Manchester, New Hampshire when employees were not protected from the hazard of being crushed. To abate this violation, the employer must ensure that all of its powered industrial truck operators are protected from this hazard, possibly by utilizing the operator restraint devices that are provided by the powered industrial truck manufacturer. Date by which violation must be abated: Immediately upon receipt of citation
