Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a) Job made ladder between 1st and 2nd floors; Employees climbed a wooden job made ladder which did not have filler blocks of the same thickness as the ladder's cleats, inserted tightly against the underside of each cleat; as required by ANSI A14.4-1979, Section 4.5.2. Additionally, the perpendicular distance from the face of the cleat on the climbing side to the nearest fixed object on the back side of same ladder was less that the minimum 10 inches required by ANSI A.14.4-1979 section 6.3. Employees using the job made ladder, were exposed to a maximum fall hazard of approximately 18 feet to the floor below. Among other methods, two feasible and acceptable methods to correct these hazards are to include, but not limited to the following: Conform to the American National Standard Institute (ANSI) A14.4-1992 section 4.5.2 and section 6.3. Violation occurred on or about 12/21/2005.
