Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:On or about 12/09/05 Location:West Side of Main Building Equipment:Aerial Tramway Condition:Employees traveling on the aerial tramway 50 feet above ground level having, but not limited to, the following deficiencies were exposed to fall and strike hazards in the event of equipment failure or damage: Operational controls were not provided in the carrier nor was an operator at the controls, maximum carrier speed was not established; drive sheave and track cable brakes, tension monitoring equipment, and a stop device in the carrier were not provided, guards were not installed on sheave units, maximum load of carrier was not established, systematic dynamic load testing was not performed, carrier and hanger were not tested against accepted criteria established by a qualified engineer, wire rope clips were not installed and spaced in accordance with the manufacturer. "Among other methods, one feasible and acceptable method to correct these hazards is to develop and implement a written quality assurance program to include preoperational and annual inspections in accordance with American National Standards Institute B77.1-1999, Safety Requirements for Passenger Ropeways--Aerial Tramways, Aerial Lifts, Surface Lifts, Tows and Conveyors--to ensure the integrity of the installation, operation, and maintenance of the aerial tramway. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
