Text For Citation: 01 Item/Group: 001 Hazard: DUST&FUMES
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputations due to inadvertantly activating the machine: (a)Shop Area. The foot pedal that activates the 225 ton capacity Cincinnati mechanical press brake, Serial No. 32906, was not guarded on or about August 12, 2008. (b)Shop Area. The foot pedal that activates the 75 ton capacity Chicago Dreis & Krump Mfg. mechanical press brake, Model 810-C, Serial No. L15972, was not guarded on or about August 12, 2008. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to guard/cover the foot pedal to prevent accidental actuation by falling or moving objects, or by someone accidentally stepping on the pedal.
