Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: On or about 03/25/06, employees were exposed to the hazard of being struck and crushed by overhead cargo containers that were being loaded onto a ship with the use of a crane, while the crane operator lacked communications and visibility with the longshoremen on the vessel. Among other methods, one feasible and acceptable method to correct this hazard is to provide radios that have batteries completely charged, instruct employees to inspect the radios prior to commencing longshoring operations, and provide employees with refresher training related with recognizing the hazards of being struck-by equipment during longshoring operations. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
