Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: On or about 10/29/05, at the hurricane debris collection site that was lacking a survey of the land and worksite features, an employee who was lacking training on operating an excavator on unstable ground was crushed by the equipment when it rolled over and fell to ground level, from an approximate height of 22-feet. Among other methods, one feasible and acceptable method to correct this hazard is to: Provide excavator operators and employees with training on equipment controls, components, their functions, and limitations; and instruct employees on how to conduct a careful survey and record of the land and worksite features, prior to operating the equipment, as outlined in the equipment operator's manual.
