Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by steam and fragments should the dryer cans fail: a. Paper machine #1 and paper machine #2, on or about 10/11/2006: No inspections of the dryer cans were performed. Failure to Inspect the dryer cans could result in a catastrophic failure where employees would be struck by steam and fragments of the dryer can. Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1. Implement a pressure vessel program in accordance with TAPPI, 2001 TIP 040216. This program shall consist of a detailed internal and external visual inspection conducted by experienced personnel, magnetic particle testing and inspection of the dryer heads conducted by certified personnel, and ultrasonic thickness testing performed certified personnel. 2.Inspect dryer cans as per Factory Mutual loss prevention manual 12-25, Paper Machine Dryer Rolls. Abatement certification must be submitted for this Item.
