Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Maintenance Department, on or about 07/25/06, a 2 ton Dayton electrically operated overhead hoist being used to lift heavy items during maintenance and/or loading equipment onto vehicles had the following observed hazards: 1.The manufacturer's hook safety latch had been removed. 2.The electrical pendant control cord did not have support to prevent strain on the cord which resulted in the insulation pulling loose at the point of connection to the hoist. 3.The hoist assembly had not been inspected and no records or inspection code marks were available for the hoist. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Replace the hoist hook safety latch in accordance with B30.16, 1993, Chapter 16-2, Overhead Hoists. 2.Reinstall the outer insulation to prevent chafing of the inner conductors and apply a method that will protect the electrical conductors against strain in accordance with ANSI B30.16 - 1993 Chapter 16-1, Overhead hoists. 2.Conduct frequent and periodic inspections of the hoists in accordance with ANSI B30.16-2, 1993, Chapter 16-2, Overhead Hoists.
