Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a) Core Room, Station 36, Osborne Roll-Over, on or about 4/27/06: Employee was operating a 1/2 ton capacity air-operated chain hoist with no inspection procedures developed and utilized for the hoist. b) Core Room, Pep Set, on or before 4/27/06: Employee was operating a air- operated chain hoist with no inspection procedures developed and utilized for the hoist. c) Core Room, Shell Area, on or about 5/2/06: Employee was operating (2) air- operated chain hoists with no inspection procedures developed and utilized for these hoists. d) Core Room, Core Assembly Area, on or about 5/2/06: Employee was operating a Gardner-Denver air-operated chain hoist with no inspection procedures developed and utilized for the hoist. e) Core Room, Dry Sand Core Assembly, on or about 5/2/06: Employee was operating a air - operated chain hoist with no inspection procedures developed and utilized for the hoist. ABATEMENT NOTE: One means of abatement would be to ensure that the air- operated chain hoists be inspected per American Society of Mechanical Engineers, ASME B30.16-1993, Chapter 16-2, Sections 16-2.1.1, 16-2.1.2 and 16-2.1.3. f) Core Room, Pep Set, on or about 4/27/06: One air-operated chain hoist capacity was not marked on the hoist or load block. g) Core Room, Shell Area, on or about 5/2/06: Two air-operated chain hoists capacity was not marked on the hoist or load block. h) Core Room, Dry Sand Core Assembly, on or about 5/2/06: One air-operated chain hoist capacity was not marked on the hoist or load block. ABATEMENT NOTE: One means of abatement would be to ensure that the rated load of each hoist is marked on the hoist or load block per American Society of Mechanical Engineers, ASME B30.16-1993, Chapter 16-1, Section 16-1.1 i) Core Room, Pep Set, on or about 4/27/06: The supporting structure including the monorail for the air- operated chain hoist was not designed nor rated to withstand the loads and forces imposed by the hoist for the rated load.j) Core Room, Shell Area, on or about 5/2/06: The supporting structure including the monorail for two air-operated chain hoists was not designed nor rated to withstand the loads and forces imposed by two air-operated chain hoists for the rated loads. k) Core Room, Dry Sand Core Assembly, on or about 5/2/06: The supporting structure including the monorail for the air-operated chain hoist was not designed nor rated to withstand the loads and forces imposed by the hoist for the rated load. ABATEMENT NOTE: One means of abatement would be to have a qualified person review the design of the supporting structures and establish rated load capacities as per American Society of Mechanical Engineers, ASME B30.16-1993, Chapter 16-1, Section 16-1.3.2. l) Core Room, Pep Set, on or about 4/27/06: Below-the-hook lifting devices used to support molds and attach loads to an air-powered chain hoist were not marked as to their rated capacity. m) Foundry Dept., 1175 Head Line, on or about 5/3/06: Below-the-hook lifting device used to lift and support degasser unit and attach loads to an air powered cain hoist was not marked as to its rated capacity. n) Foundry Dept., 1175 Head Line, # 2 Clam on or about 5/3/06: Below-the-hook lifting device used to lift cylinder head molds with molten metal was not marked as to its rated capacity. ABATEMENT NOTE: One means of abatement is to have a qualified person review the design of the below-the-hook devices and have their established capacities marked on the devices per American Society of Mechanical Engineers, ASME B30.20-1993, Chapter 20-1, Section 20-1.2.1. o) Core Room, Pep Set, Foundry Dept. 1175 Head Line, on or about 5/3/06: Below- the-hook lifting devices used to lift and/or support molds and attach loads to air- powered hoists were not being inspected using the frequent and periodic procedures as specified in American Society of Mechanical Engineers, ASME B30.20-1993. ABATEMENT NOTE: One means of abatement would be to ensure that the below- the-hook lifting devices be inspected per American Society of Mechanical Engineers ASME B30.20-1.3.1. p) Foundry Dept., Bay 9A, on or about 5/4/06: Pendant control station for Yale five ton crane was not supported in a manner as to protect the electrical conductors against strain. ABATEMENT NOTE: One means of abatement would be to provide a wire rope and/or chain attached to the pendant control and hoist motor control per American Society of Mechanical Engineers, ASME B30.16, Chapter 16, Section 16-1.2.3(d). Abatement certification must be submitted for this item.
