Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Carbon bake plant bldg 351, on or about 12/07/2005: Two 225 Gang Tongs,numbers: 88267 A & B , #283 & 284, were being used daily to lift and move baked anodes with four crane hook retaining pins worn below the manufacturer's specifications and were not being inspected by a qualified person in that the manufacturer required the pins to be replaced at 7or greater wear and the pin was actually worn to 25without replacement or removal from service. b)Carbon bake plant bldg 351, on or about 12/07/2005: Three below-the-hook metal Dump Boxes used to carry carbon dirt to fill the baking ovens were not rated and marked with the load capacity. Among other methods, feasible and acceptable methods to correct each of these hazards are to: 1.Require a qualified person to conduct frequent and periodic inspections of the lifting devices and to replace or repair in accordance with Section 20-1, ANSI/ASME B30.20-1993, Below-the-Hook Lifting Devices and the manufacturer's specifications. 2.Rate and mark the load capacity of the dump box lifting devices on a tag/plate or equivalent.
