Text For Citation: 01 Item/Group: 001 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and or explosion: a) R Stahlman 1 gas well, Monroe Twp, Clarion Cnty, - Employees working on Ingersoll Rand RD-20 Rig #6 were exposed to an unsafe air mixture environment resulting in an explosion and fire causing injuries. The blowout preventer (BOP) had been removed and the well emitted natural gas vapors for approximately two (2) hours before an attempt was made to cap the well with a swedge fitting.
