Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by/caught-between hazards and related crush injuries from falling loads that were suspended by below the hook lifting devices (e.g. spreader bars) and cranes: A.) Burn Table End/South End Crane - --- Yellow Colored Spreader Bar/(Spreader Beam) -- Marking & Labeling: As of April 19, 2007, the spreader bar was not marked with a rated load capacity, safety warning data, and its weight (i.e. the weight of the device itself). The spreader bar was utilized in conjunction with 10-ton North American Cranes and in the Burn Table Area to lift sheets of steel and occasionally "skeletons". (Reference: ASME B30.20 Below the Hook Lifting Devices Section 20-1.2.1) Abatement Note: Among Other Methods, Feasible and Acceptable means to correct the conditions noted above include utilizing a durable substance or device to mark the Spreader Bar with the following information: (1.) Rated load capacity (2) Spreader Bar Weight (3) Safety Warnings noted in ASME B30.20 Below the Hook Lifting Devices Section 20-1.2.1 (d) B.) Burn Table End/South End Crane - --- Yellow Colored Spreader Bar/(Spreader Beam) -- Lifting Device Inspections: As of April 19, 2007, A documented inspection program has not been established for the spreader utilized by the Linesville, PA location. Only, daily before use visual inspections were being accomplished by the operators. The spreader bar was utilized in conjunction with 10-ton North American Cranes and in the Burn Table Area to lift sheets of steel and occasionally "skeletons". (Reference: ASME B30.20 Below the Hook Lifting Devices Section 20-1.3.6) Abatement Note: Among Other Methods, Feasible and Acceptable means to correct the conditions noted above include creating a documentation program for the spreader bar inspections. The inspection record should document critical items such as (1) Loose components, connections, or fastners (2) Chains (3) Hooks (4) Linkages (5) Wear at hook points, clevises, or pins etc. (6) Beam Sections for Deformation (7) Welds. The inspection record should document the date of inspection, person performing the inspection, and required corrective actions. C.) Burn Table End/South End Crane - --- Yellow Colored Spreader Bar/(Spreader Beam) - Training: The spreader bar was utilized in conjunction with 10-ton North American Cranes and in the Burn Table Area to lift sheets of steel and occasionally "skeletons". Personnel had not been formerly trained on the Spreader Bar's operation, use, inspection, and limitations noted in ASME B30.20 Below the Hook Lifting Devices. (Reference: ASME B30.20 Below the Hook Lifting Devices Section 20-1.4.2) Abatement Note: Among Other Methods, Feasible and Acceptable means to correct the conditions noted above include developing and instituting a formal training class for employees on the Spreader Bar and other below the hook lifting devices. The training class should cover at a minimum the training criteria noted in ASME B30.20 Below the Hook Lifting Devices Section 20-1.4.2
