Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to a point of operation hazard because the two-hand trip device was not protected from the unintended or inadvertent operation: Production department; The palm button control station (no model or serial number) used to cycle the load side of the Dieffenbacher No.10-202 2500MT hydraulic press (model #2500 ton, serial #E-109051.01.001.1) was not anchored to the floor to prevent the inadvertent or unintended cycling of the press in the event the palm button station was ever accidentally knocked over. Employee(s) were exposed to the hazard of being crushed by the press. Among other methods, one feasible and acceptable method to correct this hazard would be to anchor the palm button station to the floor. DISCLAIMER 1.
