Employer had not established operator training or documentation of training; periodic inspection procedure of the lifts; lifts locks were not working on all lifts and stands were not provided; a preventative maintenance procedure was not established; repair documentation was not maintained; and manufacturer's hazard warning decals were not available for employees, on or about 8/29/05.
