Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads and falling hoist parts: a) Flabeg Corporation, 851 West Third Avenue, Brackenridge, PA: On or about 01/12/2006, the hoist located in the Glass Room was not kept in good repair in that the following deficiencies were noted: (1) Frequent and periodic inspections were not conducted; (2) a preventive maintenance program was not implemented; (3) the running rope was kinked; (4) the trambeam was rated for 3 tons, and the lower hoist block was not marked with its rated load capacity. Among other methods, one feasible and acceptable abatement method to correct this hazard is to develop and implement a preventive maintenance program which incorporates all recommendations outlined in the manufacturer's manuals with written, signed, and dated reports and also in accordance with paragraph 11.2 of ANSI B30.16-2003 and paragraph 16.2.2 of ASME B30.11-2004.
