Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by and struck by earth moving equipment: a) B Pocket - Management does not effectively enforce the use of seatbelt which are equipped on earthmoving machinery. The operator of the Caterpillar Track Type Loader #501129 was observed not wearing the seatbelt. One feasible means of abatement would be to ensure that operators wear the seatbelt in accordance with the company's Safe Work Procedures #13 which states the following: "Wear PPE: Seat Belt..." b) Outside Yard - The Euclid 500000 dump truck had an inoperative reverse signal alarm. One feasible means of abatement would be to ensure that all safety devices are maintained in an operable condition. Observed on or about 1/19/06 ABATEMENT CERTIFICATION REQUIRED
