Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of Public Law 91-596, the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards: due to possible overloading of hoists and/or loads being dropped due to the absence of rated loads for "Below the Hook Lifting Devices" not having been conspicuously displayed on the device(s). No inspection of the devices were being conducted and documented at required times and/or intervals: (a)On or about January 25, 2007 to February 8, 2007 at the worksite there were no rated load markings visible on the main structure and no identification plates which listed: 1. manufacturer's name and address, 2. serial number of the device, 3. lifter weight if over 100 lb (45 kg) 4. rated load on the "Below-The-Hook Lifting Devices attached to the monorail hoists in the: (1) Frame Shop, (2) Floor Shop and (3) Wall and Roof Assembly areas. [Reference Section 20-1.2.1(a) & (b) of American National Standard, ASME B30.20b-1996, Below-the- Hook Lifting Devices.] (b)On or about January 25, 2007 to February 8, 2007 at the worksite the Below-the- Hook lifting devices being used in the Frame Shop, Floor Shop and Wall and Roof Assembly areas had not been inspected nor had records of required inspections been maintained. [Reference Section 20-1.3 Inspection of American National Standard, ASME B30.20b-1996, Below-the-Hook Lifting Devices. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: (1) stencil a clearly visible load rating on each "Below-the-Hook" lifting device main structure, (2) affix an identification plate to each "Below-the-Hook lifting device which includes the information required by (a) 1,2,3,& 4 above and (3) conduct and document inspections as required by Section 20-1.3 of ASME B30.20b-1996.
