Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm such as amputation or permanent disability when struck by unintentionally released attachments from the excavator arm. On 04/15/2005, The excavator bucket fell from the Kobelco hydraulic excavator which was using a Werk-Brau quick coupler device to secure the bucket in place on the excavator arm. Among other methods, the following feasible and accepted abatement methods to correct this hazard are: Follow the manufacturer's recommendations for installation, operation, inspection and maintenance of the quick coupler device to insure that the excavator attachments are not released from the coupler unintentionally.
