Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: Facility located at 1009 W. M St. Hastings, NE - The employer is not furnishing employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious harm to employees in that employees are exposed to fall hazards when they are working on a pallet being elevated in the air by a powered industrial truck. The employer is failing to ensure that employees are protected from a fall hazard when they are being elevated on a pallet connected to a powered industrial truck. Employees were observed by management working from the elevated pallet. Employees are not being provided a safe, secure platform or any type of fall protection while they are working in this manner. Among other methods, feasible and acceptable methods of abatement are: 1) Provide a manufacturer approved "man basket" to be used when employees need to be elevated to do work in the facility. 2) Develop and implement written procedures requiring workers to work only from an approved "man basket" whenever they have to be elevated to perform work. 3) Develop and implement a disciplinary action and procedures to ensure that employees utilize an approved "man basket" whenever they have to be elevated to perform work. 4) Ensure that all operators have received a certified powered industrial truck training course prior to them being allowed to operate the vehicles in the facility. This training should meet or exceed the requirements of 29 CFR 1910.178(1)(3). Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.ion
