Section 5(a)(1) of the Occupational Safety and Health Act of 1970: Jobsite 4911 S. 72nd St., Omaha, Ne - The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falls while disassembling and removing warehouse shelving. Employees were elevated on the platform of a powered industrial trucks without proper fall protection. Among other methods, feasible and acceptable method to correct this hazard includes, but not limited to the following: i)Implement the precautions for using an industrial truck to elevate personnel as outlined in ASME Standard B56.1-2000 paragraph 4.17.2. ii)Develop and implement written procedures for the safe elevation of personnel using powered industrial trucks per ASME Standard B56.1- 2000. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
