Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: fall hazards Worksite located at 1800 Harrison Ave., Kearny, NJ a) Employees torch cut the tops of electrical transformers they used as work platforms and anchorage of personal fall protection. b) Employee used extended Genie Scissor lift as anchorage for personal fall protection while taking combustible atmospheric readings of the transformer. Violation observed on or about 5/2/05 ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THIS HAZARD IS: 1. Provide employees alternate work platforms to perform transformer top cutting, such as but not limited to, a scissor lift OR 2. Provide employees with proper anchorage for personal fall arrest system, such as but not limited to, an adjustable personnel hoist AND 3. Do not allow employees to tie-off to the Genie scissor lift per the operator's manual.
