Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: Sheet Metal Receiving: The R & M overhead crane, No. 20, had the following deficiencies: 1. The maximum rated capacity of the loader spreader/sheet metal grabber did not match with the maximum rated capacity for the crane. 2. The hook attachment ring of the spreader was too big for the hook of the crane, causing damage to the throat of the hook of the crane. 3. The sizes of the sheave grooves of the load block did not match with the size of the wire rope installed, causing damage to the inner face of the grooves and rope. 4. No frequent and periodic inspections done, including daily visual inspections or routine maintenance. Among other methods, one feasible and acceptable abatement method to correct the hazard is to: 1. Install the crane and sheet metal grabber with matching load ratings. 2. Install the crane and sheet metal grabber with proper fitting hook and hook attachment. 3. Install the load block with wire rope that properly fits the sheave grooves. 4. Conduct periodic and frequent inspections, including daily visual inspections and routine maintenance. 5.
