Text For Citation: 01 Item/Group: 001 Hazard: LACKTRAIN
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: a)On or about 08/04/05, and at times theretofore, at 9600 Clinton Drive, Galena Park South Yard, the employer did not furnish a place of employment which was free from recognized hazards likely to cause death or serious physical harm because the employer did not a) insure that operators of boom-supported aerial work platforms were trained on the aerial platform model to be used prior to use of the work platform, b) insure that means were used to protect against use by unauthorized use, c) insure that aerial platforms were used in accordance with the operator's manual and ANSI/SIA A92.5-1992, copies of which were available on the lift, and d) insure that work rules regarding mounting and dismounting of the work platform were developed, implemented and enforced. One method of abatement, among others, is to insure operators of boom supported aerial work platforms have been trained on the model of lift to be used, insure that training and operating procedures include all provisions in the manufacturer's operation and maintenance manual, and that responsibilities outlined in ANSI/SIA A92.5-1992 are read and understood by owners and/or lessors, users, and operators. *** Certification and Documentation of Abatement Required ***
